Specifications Exclusive to a Dissertation

The "Original Copy" of a dissertation must also include the following:

 

Request for Deferment of Publication of Dissertation

(Optional). In the event a Ph.D. candidate has applied for a patent, he or she may request that the dissertation not be bound until the patent is awarded by using this form.

 

Extra Copy of the Abstract

This extra copy is needed for microfilm. Ph.D. candidates whose abstract exceeds 350 words must shorten the extra copy version of the abstract to 350 words or less for the copy of the abstract used for the microfilm. The original abstract may remain as written. The shortened version (copied on regular paper) is used only for microfilming. 

 

Extra Copy of the Title Page

An extra copy of the title page (copied on regular paper) must be supplied for microfilming. Put this copy in the "Original Copy" envelope.

 

Doctoral Dissertation Agreement Form

This form is available online in pdf format. (link below) You will need to download it, following the directions to fill out and sign the form. We need page 3 and 4 filled out and signed. Option A-1 is the preferred selection.

This form is provided by the Graduate Office at the time the defense is set up and when completed is placed in the "Original Copy" envelope.

 

Commencement Questionnaire

Though it seems like a lifetime away, we need to be able to plan for PHD attendance for the next Commencement ceremony in May. Please answer the questions in this Questionnaire, and hand it in with your final copies.

This form is provided by the Graduate Office at the time the defense is set up and when completed is placed in the "Original Copy" envelope.

 

Illustrative Material

All graphs, figures, charts, diagrams, computer printouts, photographs, and illustrations must be clear and legible. Clear permanent color or black and while reproductions are acceptable. In a dissertation the "Original Copy" is microfilmed and therefore it may not contain color illustrations. Color illustrations are allowed in the other 3 copies. Photographs should be permanently attached to acid-free paper. Computer-generated illustrations are acceptable if they are clear. The illustrative pages must meet all margin requirements. All illustrations must be located on the right-hand side page of the dissertation.
