Tax Exemption Certifications for URI
Tax Exemption is available in the following states:
- Rhode Island (provide certificate to vendor/supplier)
- Florida (provide certificate to vendor/supplier)
- Maine (complete and provide certificate to vendor/supplier)
- Massachusetts (complete ST-5 and provide with ST-2 letter & original ST-2 to vendor/supplier)
- Michigan (complete and provide certificate to vendor/supplier)
- Missouri (provide certificate to vendor/supplier)
- New Jersey (complete and provide certificate to vendor/supplier)
- Ohio (complete and provide certificate to vendor/supplier)
- South Carolina (complete section 5 and provide certificate to vendor/supplier)
Most vendors will allow Accounts Payable to decrease the invoice if tax was applied in these states. If a tax exempt form has yellow highlighting, please complete that portion and provide to vendor.
